- This course was developed in response to a recommendation from the Florida Health Care Transitions Task Force for Youth and Young Adults with Disabilities that health care professionals in Florida should have access to free trainings about transition. It is based on an on-line program that was developed by the Institute for Child Health Policy for Children's Medical Services Care Coordinators and other CMS staff. It has been expanded, to include information and examples relevant to all types of health care professionals: including physicians, nurses, social workers, dentists, psychologists, mental health professionals, and nutritionists, as well as respiratory, occupational, physical, and speech and language therapists. In keeping with the recommendations of the Task Force, this course is designed to provide an understanding of how and why to transition adolescents with disabilities and chronic health conditions from pediatric care to adult oriented care. The course addresses legal, social and developmental aspects of health care transition and suggests ways to facilitate and help youth and families plan for transition. The course is composed of narrated power point presentations, and also provides suggestions for additional readings and identifies web-based resources.
